Please note: Applications should be completed and submitted after the CRNP/CNM has met with the collaborating physician and completed a collaborative practice agreement and the physican has paid the required fees to the Alabama Board of Medical Examiners (ABME) and has submitted the required form. The practice agreement requires both signatures (CRNP and Physician) and is kept on file at the practice site. The Collaborative Practice application is considered incomplete and a temporary license will not be issued until the ABN has been notified by ABME that the physician has paid all collaborative practice fees due to the Board of Medical Examiners and submitted to the Board of Medical Examiners the appropriate form [ABN Administrative Code, § 610-X-5-.04(1)(c)].

Applications are to be completed by the CRNP/CNM ONLY. As part of the on-line application the CRNP/CNM are required to complete regulatory questions and to personally attest to the accuracy of the answers. It is not acceptable for credentialing, office staff, or others to complete a collaborative practice application and may result in discipline of the nurse's license.
