You may download either the Word Document or an Access database (if prompted, choose save file to my computer) and enter the information required.
The list should be submitted to the Board of Nursing no later than January 31, 2013. We prefer the Access database format since it allows us to electronically check license numbers but we will accept other documents if in the correct format.
